Patient Information

We welcome you as a new patient to the Allergy and Asthma Center of Austin.

Your new patient appointment will last 30 to 45 minutes. To help make your time with Dr. Howland or Dr. Lieberman more productive, we ask that you complete a demographic form and a 4-page medical questionnaire before your visit.

We will mail or fax your new patient form and questionnaire to you (call 512-345-7635 and tell the scheduler how you would like to receive your questionnaire), or you may click on the links below to view and print BOTH the form and questionnaire.

Please complete your paperwork and bring it with you to your appointment. We ask that you do not mail or fax back completed forms prior to your appointment. Because these forms will become part of your permanent medical record with our office, we ask that you print them on clean, white paper for optimal legibility. The patient history form should be printed on four separate sheets of paper.

Please arrive 15 minutes before your appointment time if you have filled out your paperwork, or 30 minutes before if you have not. We make every effort to see patients on time with little or no waiting. Arriving early with your paperwork completed helps ensure that you and other patients will be seen in a timely manner.
